Step 1 · Start with your insurer

Your insurance-call checklist

Use these questions and CPT codes when you call your insurance company. Write down the answers, then contact our team if you need help with next steps.

Call your insurance company

Call the member-services number on the back of your insurance card. Tell the representative you are considering outpatient TMS at Crane River TMS in Grand Island, Nebraska, and ask about these CPT codes:

CPT codeService to ask about
90867Initial TMS treatment, including mapping and individual stimulation settings.
Usually 1 session at the start of treatment.
90868Subsequent TMS treatment sessions.
Insurance commonly authorizes 30–36 sessions, depending on your plan and approval.
90869Reassessment of stimulation settings with subsequent TMS treatment.
Performed during the treatment course if re-mapping is needed.

Questions to ask

  • Does my specific plan cover these TMS services?
  • Is Crane River TMS in-network for my plan?
  • Do I need a referral or prior authorization?
  • What previous treatments and records are required?
  • Are there limits on the number of covered sessions?
  • What deductible, copay, or coinsurance would I owe?
  • If the clinic is out-of-network, is a single-case agreement an option?

Write down the representative’s name, call date, and reference number. Benefits information is not a guarantee of payment; your plan’s requirements and the services actually provided determine coverage.

Out-of-network? Ask about a single-case agreement

A single-case agreement is an arrangement between an insurer and an out-of-network provider for a specific patient’s care. It may allow treatment to be covered under agreed terms when the plan permits it.

Our team can help review whether this option may be available and coordinate with your insurer when appropriate. Approval is not automatic, and coverage, reimbursement, and your share of the cost depend on the insurer’s decision and the written agreement.

Ask your insurer whether it considers single-case agreements for TMS and what documentation or authorization is required. Any agreement should be confirmed before treatment begins.
