Quick reference for referring clinicians
One-page TMS referral guide
Use this overview to prepare a referral. Our provider makes the final clinical and safety determination after intake and evaluation; coverage requirements vary by plan.
1. Who to consider
Adults with major depressive disorder who have not had sufficient improvement with prior treatment may be candidates. Include the diagnosis, current symptoms and functional impact, treatment goals, and relevant previous medication and psychotherapy history. A referral alone does not establish eligibility. Flag seizure history, head injury, implanted metal or electronic devices, and other safety concerns for individual review. The TMS clinician will assess the specific history and device information.2. Records to send

- Recent assessment and clinical notes supporting the diagnosis
- Antidepressant trial details, dose, duration, response, and tolerability when available
- Psychotherapy history and recent symptom score, such as PHQ-9, when available
- Relevant medical history and current medication list

4. What happens next
After receiving the referral, our team reviews available records and contacts the patient for intake questions. A Crane River TMS provider evaluates clinical suitability and safety. We then review benefits and any authorization requirements before scheduling mapping, if appropriate. View Referral Form PDF below ↓View Referral Form PDF Open preview ▾
